By completing this form, you give Army Emergency Relief permission to start a contribution allotment from your monthly active duty or retired pay. Defense Finance and Accounting Service (DFAS) has approved this process. Please check your DFAS statements to verify deductions for the year.
Include your e-mail address if you wish to receive electronic acknowledgement of your donation, or you can print a copy of your allotment pledge on the final screen.
